Fort Worth ISD Family Action Center
The Fort Worth ISD Family Action Center (FAC) is located at 5500 Norris Street adjacent to Sunrise McMillan Elementary School and the Stop Six-Walter B. Barbour Health Center JPS Health Center. The Center also operates a satellite office at 5701 Meadowbrook, Building 3, on the Eastern Hills High School campus. Who are we?
We strive for a robust delivery of client wraparound services and referrals, primarily for eastsiders, focusing on behavioral and physical health services, as well as community resources for food, shelter and job advancement and security. The focus is on students and families as well as individuals living in east and southeast Fort Worth, particularly those within the Eastern Hills and Dunbar high school attendance zones. We do this by rolling up our sleeves and working hand-in-hand on a grass-roots level with individuals, families, schools, organizations, non-profits and faith-based groups to identify synergies and partnerships that can impact our students' well-being and academic outcomes.
What are we?
The FAC staff adds valuable community resource assistance along with behavioral and mental health needs. The Center is unique in that it serves students, families and individuals who are not necessarily current FWISD clients with the premise that a strong, supportive and sustainable community creates stronger, more successful students. This includes pairing people up with services for rent and utility assistance programs, job openings, adult education training opportunities, food assistance, housing assistance, clothing and toiletries, Children's Health Insurance Program (CHIP) applications and more.
Bringing knowledge to the people.
The FAC also offers online and in-person classes and seminars -- called FAC-e Time Conversations -- to help uplift, educate, inform, and, more importantly, empower people in the community to make positive changes in their lives. We invite subject matter experts to join us to provide the most up-to-date information designed to uplift and inform people who want to make positive changes.
Power of Partnerships.
We partner with co-located service providers who provide help to students and adults. Some of those services occur at the FAC while others may happen at the providers' facilities. Links to frequently used co-located service providers can be found here. We are constantly adding new partnerships, so visit often to learn about new services. Also, the FAC's clinical staff connects residents, families and students to the resources they need to obtain mental, emotional, physical health and community resources.

If you are a parent, teacher or administrator who needs help finding mental, physical or community services, please fill out this form:
Once a form is submitted by either a parent or a service provider/school staff, you will be contacted by a clinician to discuss your specific needs and next steps within the next business day.
The Family Action Center oversees a Care Closet for clothing and toiletry support to provide a hand-up to community members and students who are lacking those resources, especially those who are homeless or in transition.
Clients can make appointments to access the Care Closet, located on the Eastern Hills High School campus.
Donations of new or gently used clothing and financial contributions are always welcome.
