Please read the instructions on the home page carefully. Please download and print the official UIL Medical History/Physical Exam form for your health care provider. All student-athletes in grades 7th - 12th in Fort Worth ISD must undergo an annual physical examination. The physical examination must be dated after May 1, 2024, to be considered valid for the 2024-2025 academic year. Please note that all physicals expire at the end of each school year, regardless of the examination date.

High school student-athletes are required to submit their completed physical form to the athletic trainer, while middle school students must provide their form to the coach of their first sport. A new physical will be necessary for the upcoming school year.

By completing these requirements, student-athletes will be prepared to participate in the Summer Strength/Conditioning Programs & Sport Specific Skill Instruction as well as Athletics for the 2024-2025 school year.
